SparksMD is currently accepting new patients! Enrollment is simple, and your membership provides ongoing access to personalized primary care built around a strong, long-term relationship with your physician, with more time, better access, and continuity of care.
There is no charge when you complete the enrollment form. During enrollment, you will select your preferred monthly billing date. Your first membership payment will be charged on that date or prorated at your first visit, whichever comes first. For a limited time, we are waiving our enrollment fee for new patients.
